At anytime during the process of creating and/or saving a “Forecast” you have the option of returning to the “Intervention” tab of this Site and opting out of CHCS review. If you choose to opt-out, CHCS will not have access to any of the forecast information you input or save to your account.

CHCS also collects aggregate data on Web site user sessions, such as the number and frequency of visitors to this Web site. This information may include the Web sites that you access immediately before and after your visit to our Web site, the Internet browser you are using, and your IP address. If we use such data at all, it will be on an aggregate basis, and we will not disclose to third parties any information that could be used to identify you personally.
